We're inviting all Community Champions to take part in this survey which should take around 10 minutes to complete.

The aim of this survey is to understand what Champions like about the initiative, what could be improved and your thoughts about future focus and activities.

Answers are anonymous and will only be used for the future development of the initiative.

To allow more people to complete the survey, we have extended the deadline to 17 October.
